Actually one that comes to mind was Leg 11 of The Amazing Race Latin America 3

In Salvador, Brazil, one of the Detour choices involved teams holding coconuts with their bare hands and using a huge knife with their other hand to break the coconuts open and empty the juices inside.

Only one team took this choice (Rick & Kathy), but every time Rick swung the knife I winced and shivered. All it would have taken was a tiny little miscalculation, and Rick would have lost a finger.

I honestly couldn't believe the lack of safety.
